Post-Graduation Survey

A Youth Facilitator will call Holland Bloorview graduates 3-6 months after their last medical appointment to offer a survey. The survey will ask about your transition-to-adulthood experience at Holland Bloorview. We greatly value youth and family feedback and use it to further improve transition-to-adulthood supports and services.
