Group events that helps clients and/or families prepare for the transition to adulthood. The events are about different topics. They are co-hosted by Holland Bloorview and an adult agency, for gradual introductions to new service providers.

Welcome - A Place to Start
Are you beginning your transition to adulthood journey? Join the Bridging to Adulthood team and a Family Leader for an introduction to various community supports and resources that can help you through your journey into adulthood. We will provide a brief overview of funding supports, legal considerations, employment, recreation & life skills, primary care, post-secondary, independent living, and much more.
We welcome clients age 14 and older, along with their families.
Participate in a Q&A with a Family Leader and the Bridging to Adulthood team.

Would you like a personal one-to-one meeting with the Bridging to Adulthood team? Here we can directly answer your questions, discuss goals, and provide support with the next steps on your path into adulthood. Meetings can occur virtually, over the phone, or in-person at Holland Bloorview.
This is a 1 hour appointment.
