Support for moving from children’s healthcare (ending at age 18) to adult healthcare (beginning at age 19).

Holland Bloorview Medical Transition Clinic Support

Holland Bloorview offers medical transition clinics for youth with specific disabilities and/or supports are built into pre-existing Holland Bloorview clinics.

Medical Transition Clinic Support

Acquired Brain Injury

Clients in Ontario may be referred from their Outpatient Brain Injury Rehab team to the LIFEspan Service at UHN-Toronto Rehab.  

Autism Spectrum Disorder or complex behavior (only if followed by Psychopharmacology Clinic)

The Psychopharmacology Clinic will assist current clients with the transition to adult medical services. No new referral required.

Cerebral Palsy

Clients at Holland Bloorview may be referred by their health care team member to the LIFEspan Clinic.  The LIFEspan Clinic focusses on engaging youth and families to prepare for transition to adult services.   

Cleft Lip and Palate/Craniofacial

The Cleft Lip and Palate/Craniofacial Team will assist current clients with the transition to adult medical services. No new referral required.

Neuromuscular Disorders

The Neuromuscular Clinic will assist current clients with the transition to adult medical services. No new referral required.

Spina Bifida or Spinal Cord Injury Outpatient

The Spina Bifida and Spinal Cord Injury Outpatient Clinic will assist current clients with the transition to adult medical services. No new referral required.
