The Family Navigation Hub aims to share information and resources with families related to social needs such as stable housing, food and income security, access to childcare and more. 

The Family Navigation Hub works with Master of Social Work students completing practicums to support the delivery of Family Navigation Interventions using a student led environment.

Holland Bloorview is providing registered active clients an opportunity to participate in a social needs screening process to help us better understand their social needs. This is a voluntary process where families may be asked to participate in completing a questionnaire that helps identify any areas of social need.
Eligibility criteria: You must be a current Holland Bloorview client and be referred by a participating Holland Bloorview provider.
