Come meet learn about March of Dimes Canada’s Learning Independence for Future Empowerment (LIFE) program and register on the spot. You will also meet transition to adulthood staff and have the opportunity to start/continue you own plan for the transition to adulthood.

Create a personalized binder or digital document with a health summary and key medical reports that you can use when going to appointments with new providers, and in case of urgent situations. Learn about and register for Connect2Care and other non-Holland Bloorview health records processes. Pull your medical records on-the-spot and ask questions directly to our friendly health records staff.
