Safehaven and Holland Bloorview launch first phase of community-based transitional care program

Partnership provides personalized, integrated care for children with complex care needs

Holland Bloorview Kids Rehabilitation Hospital and Safehaven have launched a unique new transitional care program that includes four community-based beds.  Two beds are currently open and will be followed by an additional two beds in the coming weeks.

“We are thrilled to see the launch of this transitional care program with the opening of these dedicated beds at Safehaven,” says Susan Bisaillon, CEO, Safehaven. “Staff at both Safehaven and Holland Bloorview have been working hard to develop this program since last May when the program was formally announced and we are excited to welcome the first clients to Safehaven.”

The Government of Ontario provided $3.4 million over three years to fund this program for children with medical complexity for whom the transition from lengthy hospital stays to home and community can be extremely challenging.

“This is an innovative transitional care solution that advances personalized, integrated care for individuals with complex care needs and their families, following them from hospital to the community and then home,” says Irene Andress, vice-president, programs and services and chief nursing executive, Holland Bloorview. “We look forward to supporting these clients and their families and to working together to create capacity in the children’s health care system.”

Holland Bloorview and Safehaven expect the program will help alleviate system pressures from ‘alternate level of care’ patients who are in only in hospital because they can’t be safely transitioned to home or to community.

This transitional program works to provide safe, community-based care for these clients, as well as provide education and support for families to eventually care for their children at home, increasing the quality of life for the entire family.
