Help us create client journey maps to improve your experience at Holland Bloorview

Attention clients and families!

Holland Bloorview is currently undergoing a client journey mapping initiative to help us create personalized pathways for every child, youth and family we serve. The goal of this initiative is to enhance the client and family experience at Holland Bloorview by improving the way we deliver and coordinate services and care.

We are currently in the second phase of this initiative. Many of our clients and families were involved in the first phase and helped us to create the “current state journey maps”, which outline the current journey clients and families experience. Thank you!

We are now ready to co-create a blueprint for the ideal or future state client journey. We need your input on what this could look like. To co-create the “ideal state journey maps” we would like to invite you to participate in the following workshops:

  • Inpatient specific workshop:

    We are looking for families with experience staying with us as an inpatient to participate in this workshop. To streamline this exercise, we will provide starting point maps to refine and supplement through workshop exercises.
    Tuesday, July 17, 2018 from 1 to 2 p.m. in Holland Bloorview’s conference centre (1E201)

    Wednesday, July 18, 2018 from 10 to 11 a.m. in Holland Bloorview’s conference centre (1E201)

  • Outpatient specific workshop:

    We are looking for families who have children who are active or recent outpatient clients with us. To streamline this exercise, we will provide starting point maps to refine and supplement through workshop exercises.
    Thursday, July 26, 2018 from 4:30 to 7:30 p.m. in the Blooorview School activity room (1W150)

  • Validation workshop:
    This workshop will help us to finalize the “ideal state journey maps” based on the information from the previous sessions and interviews. We are hoping to have families who participated in the earlier inpatient and outpatient specific workshops attend the validation workshop, if possible. However, families are welcome regardless of their participation in a previous workshop or interview.
    Wednesday, August 8, 2018 from 4:30 to 7:30 p.m. in Holland Bloorview’s conference centre (1E201).

Can’t attend at these times?

If you are unable to attend the workshops a 30 minute, in-person or over-the-phone interview can be arranged during the weeks of:

  • July 9 – 13
  • July 16 – 20
  • July 23 – 27
  • July 30 – Aug. 3
